Second, obtain an underwritten quote.
The size of your group will determine what is needed to get a quote.
If your group has less than 10 employees, is a virgin group, or has low participation, you must complete the following forms:
1. Census Form for a Manufacturer Group or Census Form for a Wholesale/Retail Group
2. Employee Applications
3. Heath Addendum
If your group has more than 10 employees, you must complete the following forms:
1. Census Form for a Manufacturer Group or Census Form for a Wholesale/Retail Group
2. Provide a tax id number within the “notes” sections of the quote form
Once the information is fully completed, please email the information to CentralSub@uhc.com. Please note, that this process can take 7-10 days for processing.
